Connecting circuits between DSLAM and standard cab

From what I've gathered, when fibre is ordered, a circuit is made between the DSLAM and the standard cabinet. However, as the standard cab just has the cabling coming from the telegraph poles connecting to thicker cables that go to the exchange, how would the connection work? Do they literally just connect the DSLAM pair to the pole pair to the exchange pair?

I apologize if this is a stupid question, just saw some work being done at a standard cab and wondered how they do it!
 
I would think that a copper cable runs between the old cab and the new FTTC cab. A jumper between the d side of the old cab would connect to the cable that runs to the new FTTC to make the connection to the d side equipmebt in the new FTTC.
The old e side would be left terminated in the old cab.
 
Yeah, having looked into VDSL DSLAMS, that does make sense, as they support voice, so the e side in the PCP coul dbe disconnected.

No the E side is not left disconnected. It goes from E side (pair from the exchange in the existing PCP) then to the D side pair (tie pair that goes to the DSLAM), through the DSLAM and back out on a E side pair at the old PCP from the DSLAM. This then connected to the D side pair going to the customers premises with the dial tone from the exchange but connected to the DSL signal at the DSLAM.

This is why it adds extra length onto the line (the distance to the DSLAM, then back to the PCP).
