Organisation has cable broadband supplied by (then) Telewest business broadband with a static IP address.
Virgin warned of work being carried out early morning, but what they didn't say was the static IP address would change to a completely different range!. We had to phone them to find out the change is permanent!
Now, the router was anyway configured for dynamic WAN address so outgoing internet access and Emails just used the new address. Incoming SMTP email didn't get through naturally until the DNS MX was changed, once we could find out what Virgin had done.
But what about all the VPNs and home workers who dial-in on the previous IP address? yes, these shouldn't be using a hard-wired IP address but, then again, why would you expect your static IP address to change?
